CCD Guideline & Consent - Permission for Parishes to Deliver Online Content to Students and Parent Acceptance of Internet Safety Responsibility
During school closures, St. Thomas Syro-Malabar Catholic Diocese of Chicago makes efforts to provide families with student assignments and other instructional materials through various delivery methods, including online. This form allows parents/guardians to grant permission for St. Thomas Syro-Malabar Somerset, NJ (school) to deliver this online content with parents for delivery to their children and recognize schools' responsibilities under the Children's Online Privacy Protection Act.
St. Thomas Syro-Malabar Church Somerset, NJ will follow to its best ability to take responsibilities for preventing students from accessing obscene or harmful content by personal sharing or internet connections within the school’s premises.
The protection measures block or filter Internet access to pictures that are: (a) obscene; (b) pornography; or (c) harmful to minors (for computers that are accessed by children).
Additionally, our internet safety policies include monitoring the online activities of minors. The measures also provide for educating minors about appropriate online behavior, including interacting with other individuals on social networking websites and in chat rooms and cyberbullying awareness and response.
During our virual classes, we would require our students and teachers to follow the guidelines defined in our faith formation program diary for 2021-22.
By signing this form, parents/guardians permit the St. Thomas Syro-Malabar Somerset, NJ to deliver instructional content directly to students with parents copied.
Parents/guardians acknowledge their acceptance of the responsibility of their students' internet safety and commit to the following:
1. Having a trusted adult present and monitoring student who use the internet assignment and other instructional materials, or
2. Using tools such as parental controls and other filters to protect students' internet access, as described above.
Health Check form submission before in-person Session
You need to complete a health check form before attending the in-person session scheduled for your group. Any child with symptoms meeting exclusion criteria requires a negative COVID test OR a ten day isolation AND medical clearance to attend in-person sessions. We have the clearance forms available in our ccd website. Students with a chronic or diagnosed illness/condition that display any of the symptoms should have their doctor provide documentation for review. The health form will be available in the ccd website or your child's google classroom.
Any of the symptoms mentioned as part of Health Check form could indicate a COVID-19 infection in your child and may put your child and others at risk for spreading illness to others. By consenting this, you agree to submit Health Check form prior to your in-person sessions.
It is our intention to have every children attend the scheduled in-person sessions except in case of situations above.
Faith Formation Registration Fee per family is $125.00
